Most gaps are related to missingsystematic root-cause management and weak organizational learning acrossshifts and teams. The highest leverage improvements would likely come fromstrengthening escalation routines, integrating preventive maintenance findingsinto CIP activities, and establishing consistent cross-shift learningmechanisms.
